INSTRUCTIONS (Form included in registration booklet):
Print name in registration space, list title code if applicable, check event boxes to left of name for each registrant, tally totals for each event at bottom of each column, calculate amount owed for each event, total at lower right. Please make checks and share drafts payable to: OCUL Services Corporation. Payment may also be made via credit card or your credit union's Corporate One account. (Sorry no APR charges are accepted.)
Names of Registrants
List names of all registrants. Line 1 is reserved for the Chief Executive Officer/Manager. Line 2 is reserved for the Chief Elected Officer. Include in parentheses the name you want printed on your convention name badge, if it is different than your full name [ex. Robert (Bob) Jones].

The Super Saver Registration is the best registration deal, and includes admission to all events marked with the "SS" icon, including the Opening Ceremony, all General Sessions, all Education Sessions, the Exposition Hall, the OCUL Business Session, the Thursday and Friday evening activities, the Friday Awards Luncheon, the Saturday OCUF Luncheon, and the popular Saturday Banquet. If you mark the Super Saver box, do not mark the other boxes that include the "SS" icon.

C. Guest Super Saver Registration

The Guest Super Saver Registration is the best guest registration deal, and is recommended for spouses or other guests. It includes all events marked with a "GS" icon, including Standard Guest Registration with admission to all non-ticketed events, the Thursday and Friday evening activities, the Friday Guest Activity, the Saturday Guest Activity, and the popular Saturday Banquet. If you mark the Guest Super Saver box, do not mark the other boxes that include the "GS" icon.

E. Guest Registration (Standard)
Standard Guest Registration for spouses or other guests of paid registrants provides admission to all non-ticketed events, including the Opening Ceremony, the Exposition Hall, Education Sessions, General Sessions, the Thursday and Friday evening activities and the OCUL Business Session. Registration is not necessary for guests who will attend ticketed functions only (Guest Activities, Luncheons, Banquet). Registration is required to enter the Exposition Hall, Thursday and Friday evening activities and the General Sessions.

F. Small Credit Union Registration
(Less than 5 million in assets)
Small Credit Union Registration provides a discount available for one representative from affiliated Ohio credit unions with assets of less than $5 million. You pay one-third of the $180 fee; your credit union's Chapter and The Ohio Credit Union League cover the remaining two-thirds of the registration fee. Additional representatives are required to pay full registration costs. (Tickets for the Friday and Saturday Luncheon and Saturday Evening Banquet are not included and must be purchased separately.)

I. Volunteer Achievement Program - V101 Board and Management Policies
Michael Millis, of CUNA's Center for Professional Development, instructs you on using policies as decision guides, the factors influencing policies, the classification of policies, how to write policies and develop a policy manual, how to implement policies, and the benefits of policies to the board and staff. Convention General Registration is not required for VAP course.

“Cleveland — Ethnically Speaking”
Cleveland gains its unique character through the 70 different ethnic groups that call Cleveland “home.” This tour will visit several ethnic marketplaces including the famed West Side Market. Lunch will be served at the landmark Bohemian National Hall and followed by a tour of two extraordinary houses of worship in this multicultural city, rich in tradition and historically significant.

L. Saturday Ohio Credit Union Foundation Luncheon
Enjoy a lunch with keynoter Rep. Stephanie Tubbs-Jones, a member of the U.S. House Banking Committee, at the Ohio Credit Union Foundation Luncheon. Five dollars from each luncheon registration will be donated to the Ohio Credit Union Foundation.

M. Saturday Guest Activity
"Cleveland…It's a Whole New Ballgame"
What's new in Cleveland? This tour will focus on everything from the sparkling new additions to the skyline to downtown living and the public art that enhances the streetscape. Highlights will include Gateway Plaza (home to the Cleveland Indians and Cavaliers), historic Warehouse District, North Coast Harbor, Great Lakes Science Center, Rock and Roll Hall of Fame and the new Cleveland Browns Stadium. The bus tour will be followed by a delicious buffet lunch aboard the cruising Nautica Queen.
